ollaboration for Homecare Advances in Management and Practice

The CHAMP Program was the first national initiative to advance home care excellence for older people. CHAMP's goal was to make the latest evidence based tools, e-learning and expert advice easily accessible to home care clinicians, from any computer.  

 

Program Location

The Center for Home Care Policy & Research conducts scientifically rigorous research to promote the delivery of high quality, cost-effective care in the home and community. It also supports informed decision-making by policy makers, payers, managers, practitioners, and consumers of home and community-based services.

The Visiting Nurse Service of New York is the oldest and largest not-for-profit home health care organization in the country, with more than 12,000 employees caring for more than 130,000 patients in the New York metro area.
