Framework
The Framework Initiative, conducted May 2007 - August 2008, forged a ground-breaking consensus on key values, critical practice areas, and core strategies for achieving optimal home health care for older persons. The Framework is an evidence-based guide for home care quality improvement endeavors.

This cube represents the National Framework for Geriatric Home Care Excellence. Its foundation is the goals of older adults. The three sides represent what home care providers must do to achieve these goals. Cross-Cutting Principles are core characteristics of excellent care that span practice areas. Key Practice Areas are the main domains of home care. Strategies for Implementation are the actions home care providers must pursue to improve care.
