The Framework initiative was supported by a grant from The John A. Hartford Foundation. Founded in 1929, the Foundation is a committed champion of training, research and service system innovations that promote the health and independence of America’s older adults. About the Framework
The Framework Initiative established the first national consensus on key values, critical practice areas and core strategies to shape home care for older adults.
Home care is primarily geriatric care, since most of the patients that use home care services are over the age of 65. But until this initiative, no national, overarching framework existed for the provision of high quality, effective home health care to older adults.
Project Overview
This initiative (May 2007 - August 2008) was guided by a National Advisory Council consisting of leaders from home care, geriatrics, nursing, medicine, social work, pharmacy, paraprofessional services, and from consumer and caregiver organizations. Council members reviewed evidence from:
- In-depth literature reviews
- Commissioned papers
- Three regional practice focus groups
Informed by these sources, the experts developed specific practice, policy and research recommendations for six key home care practice areas and a set of overarching strategies for advancing home care excellence.
