Tuition for the Geriatric Medication Management course is $149 per participant.
Thanks to the generosity of the California HealthCare Foundation, front line nurse managers from California-based agencies now receive a discount of $40 off the tuition for CHAMP's Geriatric Medication Management Course.
Participants will receive continuing education contact hours upon completion of the course. Partial credit will not be given for partial completion of the course.
Most older home care patients are taking more than 5 medications. Medication-related problems contribute to poor patient outcomes and are a leading cause of avoidable rehospitalizations.
CHAMP’s Medication Management course is designed to give frontline home care managers the skills and tools they need to help their clinical team improve medication management for older home care patients. The course includes best practices in medication management for home care patients, including:
- Medication Reconciliation
- Identifying drug/drug interactions and medication-related complications
- Improving management of oral medications
- Communicating with physicians about medication issues
