Hebrew Hospital Home Continuum of Care based in Valhalla, NY, registered more than 70 of their frontline nurse managers, clinical directors and Quality Improvement officers for the current Geriatric Medication Management class. We asked Lauren Huber, Director of Staff Development, about the goals and objectives she has for her team:
“Our mission is to be recognized as a leader in quality and excellence in the home care industry. This commitment to meeting the needs of those that we serve leads us to consistently examine all of the areas involved in quality care, and to focus on initiatives that will support the best patient outcomes. We have identified medication management as one of the most important roles of the home care clinician/case manager as it significantly impacts avoidable hospitalizations, patient safety and care transitions. The prevalence of polypharmacy among the chronically ill population further requires thorough and complete medication reconciliation/management, including not only the patient but their family and caregivers. In recognition that effective medication management ensures the intended medication benefits, that patients are at decreased risk, and the increased likelihood that patients will remain in the community we felt that focusing on this area for clinician education at all levels was imperative.
In our experience, CHAMP programs and tools are dynamic, interactive, and promote not only education but carryover to practice – which is paramount. From any perspective be it process, quality, service or outcomes, we felt that the CHAMP Medication Management Course would be a great support to our ongoing organizational change and growth.”
