National Partners
National Association for Home Care & Hospice (NAHC)
The National Association for Home Care & Hospice (NAHC) is the nation's largest trade association representing home health agencies, hospices, home care aide organizations, and medical equipment suppliers. NAHC addresses the interests and concerns of home health agencies through educational programming, promotion of professional development and quality of care initiatives, and lobbying activities. NAHC views its partnership with the CHAMP program as an important step toward fulfilling its role in the home health community.
Community Health Accreditation Program (CHAP)
Since 1965, CHAP has established Standards of Excellence for community based care providers. Accreditation services today include home health, hospice, home medical equipment, private duty, infusion nursing, pharmacy, public health, community nursing centers and supplemental staffing. CHAP’s voluntary accreditation program provides direction and support to providers who wish to meet more than the minimum standards for service provision. CHAP is also authorized by CMS through its deeming authority to provide Medicare surveys for home health, hospice and DMEPOS organizations. CHAP’s consultative approach to the site visit process is geared at creating a learning environment in which performance improvement flourishes.

The Home Health Quality Improvement (HHQI) National Campaign is a grassroots movement designed to unite home health stakeholders and multiple health care settings under the shared vision of reducing avoidable hospitalizations and improving medication management.

Iowa Alliance in Home Care
The Iowa Alliance in Home Care is committed to promoting high quality, comprehensive, coordinated home care services for Iowans. We serve as an organized and unified voice for home care throughout Iowa. IAHC represents more than 80% of Medicare-certified Home Health Agencies, and other providers of in-home services. Membership is open to Iowa home care providers (i.e. home health agencies, home infusion, private duty, personal care services, home medical equipment), businesses that offer products and services to Iowa home care providers and individuals with an interest in the Iowa home care industry.
MHA: An Association of Montana Health Care Providers
MHA is the principal advocate for the interests of members in their efforts to improve the health status of the communities they serve. MHA's members provide the full spectrum of health care services, including hospital inpatient and outpatient, skilled nursing facility, home health, hospice, physician, assisted living, senior housing and insurance services. Every not-for-profit acute care hospital in the state is a member, ranging from the smallest critical access hospitals providing primary care services in Montana's rural communities to the largest tertiary care hospitals in the state.
Minnesota HomeCare Association
Minnesota HomeCare Association (MHCA) represents approximately 250 home care agency members who provide hands-on care to Minnesota residents in their homes. MHCA includes in its membership all types of agencies: county public health nursing services, hospital and nursing home-based programs, proprietary, and private non-profit. About two-thirds are located in greater Minnesota, and one-third are in the seven-county metropolitan area. MHCA provides a variety of services to its members including information, education, advocacy for adequate reimbursement and appropriate regulation for home care, and a code of ethics for home care.
New Mexico Association for Home and Hospice Care
The New Mexico Association for Home and Hospice Care serves its membership through advocacy, networking, education and communication by promoting success and best practices for home care, hospice and related home health support services.
Oregon Association for Home Care
